Our Community Engagement Program (CEP) is looking to hire a Case Manager II (CADC R & QMHA required). This Case Manager is a critical member of a multi-disciplinary care team providing intensive case management and integrated dual diagnosis treatment. As a CEP Case Manager, you will bring counseling services to where folks are living on the streets. You will be working with a close and experienced team of other Case Managers and Peer Support Specialists to reduce barriers for clients and obtain needed resources like housing, medical care, and mental/behavioral health care.

Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. Schedule: Monday-Friday 8:00am-4:30pm with some need to flex hours based on client need.

Location: CCC's Golden West in Old Town Portland

Sign On Bonus: $3000 hiring bonus. $1,500 paid out at time of hire and $1,500 paid after 6 months of service. (Sign on bonus not available for internal applicants).

The Community Engagement Program (CEP) is a multidisciplinary recovery model designed to meet the needs of chronically homeless individuals with co-occurring mental health, addiction disorders and/or physical concerns.

As a CEP Case Manager, you will be an active member of the community by creating relationships with clients, community leaders and resources. You will assist your clients by leveraging these relationships, scheduling appointments and providing transportation to reduce barriers to services. The goal of your work is to address all treatment needs including crisis management, substance abuse treatment, medication management, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing, accessing employment and educational opportunities.

Duties will include:

Individual Counseling and Case Management:

  • Provide trauma-informed, individual counseling to clients regarding mental health symptom management, substance use, personal and situational stressors, relationships, suicide and other risk concerns utilizing psychoeducational and evidence-based counseling practices.

Assessment and Care Planning

  • Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent.
  • Conduct and document substance use assessments, including mental status exam, ASAM level of care, DSM diagnosis, and clinical formulation.

Risk Assessment, evaluation and mitigation

  • Provide crisis intervention which may include: counseling, supporting client with de-escalating, performing risk assessments, supporting with safety planning, and referring clients to the ED or other sub-acute facilities.
  • Evaluate clients for risk of harm using agency approved tools, document identified risk including plans to mitigate. May include supporting team with Directors Custody transport holds when appropriate.

Requirements:

  • At minimum, must meet state guidelines for at least a QMHA R and be certified as a QMHA R with the appropriate board
  • Must have a minimum of a current CADC R certification and be certified as a CADC R with the appropriate board
  • Must have current CPR certification.
  • Must have three (3) years of work, education, training or experience specializing in chemical dependency and mental health treatment.
  • Must pass a pre-employment drug screen and DHS background check.
  • Spanish preferred.
  • The OAR prohibits A&D program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act.
  • Drivers license preferred
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy, and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess, or use a controlled substance or alcohol.
  • Must generally have the ability to walk, bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects, and climb stairs with reasonable or no accommodation.
  • Must pass a pre-employment drug screen, TB test, and criminal background check. This includes clearance by the DHS Background Check Unit.
